29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about May 12, 2022, the employer failed to provide a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about May 9, 2021, at jobsite located in 518 41st St., Downers Grove, IL. 60515 , employees were exposed to struck-by hazards while on the lower level when workers above were engaged in roofing activities including tear off and installation of new shingles. Head protection were not being utilized while employees engaged in this activity.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.100(a), which was contained in OSHA inspection number 1310559, citation number 1, item number 1 and was affirmed as a final order on October 19, 2018, with respect to a workplace located at 1893 Big Oak Cir., Northbrook, IL 60062.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about May 9, 2021, at jobsite located in 518 41st St., Downers Grove, IL. 60515 , employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun. Eye protection was not being utilized while employees engaged in this activity.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.102(a)(1), which was contained in OSHA inspection number 1310559, citation number 1, item number 2 and was affirmed as a final order on October 19, 2018, with respect to a workplace located at 1893 Big Oak Cir., Northbrook, IL 60062.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about May 9, 2022 at jobsite located in 518 41st St. Downers Grove, IL. 60515, employees were exposed to fall hazards in excess of 8 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1310559, citation number 1, item number 3 and was affirmed as a final order on October 19, 2018, with respect to a workplace located at 1893 Big Oak Cir., Northbrook, IL 60062.
