The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. (a) Facility- 714 Braddock View Drive, Lemont Furnace, PA: On or about May 10, 2022, all employees that were operating powered industrial trucks throughout the facility were not trained and evaluated on their powered industrial truck as specified in paragraph (l).
