29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about May 10, 2022, Luis Martinez and his employees were installing a slate rooftop onto a commercial building located at 9155 Clayton Road in Saint Louis, Missouri. The employer and their employees were working approximately 22 feet above the ground, without guardrail systems with toeboards, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access. On or about May 10, 2022, employees of Luis Martinez were installing slate shingles on a commercial building located at 9155 Clayton Road in St Louis, Missouri. The employer and employees utilized a ladder improperly. The ladder was not extended at least three feet above the upper landing surface or edge of the roof.
