Back of the building located at 2021 Stevens Street, Jacksonville, Florida 32207: On or about May 6, 2022, the employer exposed an employee to eye injuries caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while cutting plywood using saw. b. Roof area located at 2021 Stevens Street, Jacksonville, Florida 32207: On or about May 6, 2022, the employer exposed an employee to eye injuries caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using a pneumatic nail gun to sheath the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. Roof area located at 2021 Stevens Street, Jacksonville, Florida 32207: On or about May 6, 2022, the employer exposed employees to fall hazards ranging up tp 21-foot, in that fall protection was not utilized by employees while installing roof deck sheathing on a 5:12 pitch roof.
