29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May 9, 2022, on the north side of the site, employees were performing roofing work at heights greater than six feet from the lower level without fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about May 9, 2022, employees utilized a Werner Twenty-four (24) foot extension ladder on the Westside of the site to access the work area. The ladder was not extended three (3) feet above the landing or secured.
