29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about May 9, 2022, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

On or about May 9, 2022, at the above addressed jobsite, an employee was carrying lumber up the ladder, thereby exposing the employee to hazards associated with falls.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about May 9, 2022, at the above addressed jobsite, employees performing residential carpentry operations, twelve (12) feet four (4) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  A & E Carpentry was previously cited for a violation of this occupational safety and health standard  29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1527579, citation number 2, item number 1 and was affirmed as a final order on October 14, 2021, with respect to a workplace located at 12635 Berkshire Dr., Mokena, IL 60448.
