29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about May 11, 2022, employees were exposed to fall hazards while working without fall protection from a pitched roof having a ground to eave height of approximately 22 feet.  Mexican Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1320305, citation number 1, item number 1, and was affirmed as a final order on August 7, 2018, with respect to a workplace located at 20526-28 Lennon Dr., Frankfort, IL 60423.  No abatement certification or documentation is required for this item.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a)  On or about May 11, 2022, the employer did not maintain records or certification of fall protection training for employees exposed to fall hazards.  Mexican Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1) , which was contained in OSHA inspection number 1320305, citation number 2, item number 1, and was affirmed as a final order on August 7, 2018 with respect to a workplace located at 20526-28 Lennon Dr., Frankfort, IL 60423.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
