29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: Building 2403: On May 12, 2022 the employer did not ensure employee(s) working at heights approximately 10-12 feet were wearing fall protection harnesses per manufactures requirements. The "D Ring" was located at the wearers lower back and the chest strap was located at the sternum. Employee(s) were exposed to harness failure and/or reduced the equipment's ability to provide sufficient protection in the event of a fall.
