Fremont, OH: The employer failed to develop and ensure employees utilized energy control procedures prior to entering the automated pallet bander cell. Employees performing maintenance/servicing activities such as but not limited to changing plastic banding, without the benefit of energy isolation and control, were exposed to amputation/crushing hazards.  b) The Envases Group on or about May 11, 2022, at the address of 4600 Oak Harbor Rd. Fremont, OH: The employer failed to develop and ensure employees utilized energy control procedures prior to entering automated film wrap cell. Employees performing maintenance/servicing activities such as but not limited to changing rolls of stretch film, without the benefit of energy isolation and control, were exposed to struck-by hazards created by rotating wrapping arms.

Fremont, OH:  The employer did not ensure that operating employees tasked with maintenance/servicing activities at or near energized parts of the pallet bander/wrapper cells were adequately trained in the skills required for the safe application, usage, methods, and means of isolating and controlling hazardous energy sources. Employees tasked with this work on the aforementioned machine were not specifically trained to isolate energy sources to protect against amputation injuries prior to performing work at or near energized components of the cell.
