29 CFR 1926.303(b)(1): Grinding machines shall be equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section. a) On or about March 6, 2020, in the garage area of a new construction home, employee were using a Makita angle grinder (Serial number unknown) to cut flooring without the factory provided guard.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a) On or about March 6, 2020, in the garage area of a new construction home, employees were using a portable extension cord that had not been inspected for strain relief damage on plug end and damage on a section of outer shielding.
