29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about April 29, 2022, at the above addressed jobsite, employees performing framing operations from the roof and throughout the building did not use any means of fall protection, thereby, exposing employees to fall hazards of approximately twenty eight (28) feet.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) On or about April 29, 2022, at the above addressed jobsite, employees used an A-Frame fiberglass ladder, which was leaned against the exterior wall of the building, for access into the building. Employees were, thereby, exposed to a fall hazard of approximately five (5) feet.
