OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential struck by and crushing hazards from over pressurized tires and falling loads or automobile lift parts:  a. Site- On or about May 6, 2022 an employee mounting a Kenda Bear Claw EVO UTV tire onto an aluminum rim sustained a severe injury when the tire was over pressurized and the tire and rim propelled upward striking the employee. The tire was pressurized to approximately between 50 and 55 psi which exceeded the Kenda Bear Claw EVO UTV tire and the Coats X-Model Series Rim Clamp Tire Changer pressure limits for seating a bead.  b. Site- Employees were not protected from struck by and crushing hazards where four Rotary two post and one Rotary four post automobile lifts were not inspected annually by a qualified automotive lift institute (ALI) inspector.
