29 CFR  1926.52(d)(1): In all cases where the sound levels exceeded the values shown in Table D-2 of 29 CFR 1926.52, a continuing, effective hearing conservation program was not administered:  a. 643 Seneca Street - On or about May 12, 2022, employees were exposed to hazardous noise while working inside a build a box trench shielding device. The employees were operating jackhammer/air hammers to break up concrete that encapsulated the sewer pipe. The employees were not wearing hearing protection and no sampling had been done to determine the need for hearing conservation or protection.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a. 643 Seneca Street - On or about May 12, 2022, the employees were exposed to an electrical hazard from the use of an electric jack hammer that was not provided strain relief at the plug, which was then plugged into an extension cord that was not grounded.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
