29 CFR  1910.37(a)(3): Exit route(s) were not kept free and unobstructed:   a. Main Office, Southeast Exit Route from Breakroom - On or about May 12, 2022, the employer failed to keep the exit path from the employee breakroom through the main office free and unobstructed.  The door connecting the breakroom to the main office was blocked from the main office side with a piece of wood positioned from the floor to the door handle to keep employees on early starting shifts from entering the office.   b. Main Office, Southeast Emergency Exit - On or about May 12, 2022, the employer failed to keep the emergency exit and route free and unobstructed. The area was converted into a storage room and the emergency exit door was blocked and visually obstructed by a television, plant, and curtains.

29 CFR  1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted.  a. Storage Room in-between Warehouse and Breakroom - On or about May 12, 2022, the employer failed to ensure that the exit sign in the area was illuminated.  b. Dock Staging Area - On or about May 12, 2022, the employer failed to ensure that the exit sign in the area was illuminated.  c. Main Office, Northeast Hallway - On or about May 12, 2022, the employer failed to ensure that the exit sign in the area was illuminated.  d. Main Office, Northeast Hallway - On or about May 12, 2022, the employer failed to ensure that the exit sign in the area was illuminated.  e. Main Office, Southeast Storage Room - On or about May 12, 2022, the employer failed to ensure that the exit sign in the area was illuminated.  f. 29 CFR1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph(l):  a) On or about May 12, 2022, facility wide, the employer did not ensure that employee(s) who operate forklifts, reach-fork trucks, and electric pallet jacks were trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
