29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)   This violation occurred on or about May 12, 2022, at the job site located at 3943 Ford Drive, Heartland, TX; where employees doing roofing work of a new residential single-family house and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a)	 This violation occurred on or about May 12, 2022, at the job site located at 3943 Ford Drive, Heartland, TX: where employees doing roofing work on a new residential two-story single-family house while using a Hitachi nail gun.  Employees were not wearing safety glasses and were exposed to eye injuries.  JOSE CELIO PORTLL WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.102(a) (1) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1281195, CITATION NUMBER 2, ITEM 1 B THAT WAS ISSUED ON JANUARY 02, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 1820 ROUND TREE, FORT WORTH, TX 76107.   THE FINAL ORDER DATE WAS JANUARY 29, 2018.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  (a)	This violation occurred on or about May 12, 2022, at the job site located at 3943 Ford Drive, Heartland, TX, where employees doing roofing work on a new residential two-story single-family house where employees were using a Werner 20-feet extension ladder that did not extend 3-feet above the landing of the roof.
