29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about May 12, 2022, at jobsite located in 820 S. Quincy St., Hinsdale, IL 60521, An employee engaged in construction activities on floor level was not provided a hard hat. Employees at levels of height were performing roof tear-off , while the employees at ground level were  exposed to head injuries from falling objects.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On or about May 12, 2022, at jobsite located in 820 S. Quincy St., Hinsdale, IL 60521, employees were exposed to fall hazards in excess of 10  feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.    On or about May 12, 2022, at jobsite located in 820 S. Quincy St., Hinsdale, IL 60521, the employer failed to certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards in excess of 10 feet while performing roofing activities on a residential structure without using fall protection.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
