On or about May 12, 2022, at jobsite located in 820 S. Quincy St., Hinsdale, IL 60521, the employer failed to certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards in excess of 10 feet while performing roofing activities on a residential structure without using fall protection.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
