29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket, when working from an aerial lift: On May 12, 2022, Two employees in the basket of a Haulotte, Model HT46RTJO aerial lift were installing wood to the exterior surface of a commercial building located at 1228 Bryan Road, O'Fallon, Missouri. The crew was operating the aerial lift at a height of approximately 23 feet above the lower level without the use of a fall protection arresting or restraint system.
