29 CFR 1926.651(f):A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation. On or about 05/11/2022 employees at the Winstanley Construction project were not provided warning systems adjacent to an excavation. An employee lost control of a 2200-gallon water truck causing it to roll down an embankment during dust remediation activities.
