29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about May 13, 2022, an employee was working/walking at attic level of a residential house he was framing greater than sixteen feet above lower level. The employee was installing a laminated veneer lumber with no fall protection system in place to prevent him from falling.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  Location - 70 Main Street, Acton, MA.  On or about May 13, 2022, the employer failed to ensure that four employees exposed to fall hazards were properly trained. One of the employees was working at height greater than sixteen feet above lower level without fall protection.

29 CFR  1926.1051(a):  Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  Location - 70 Main Street, Acton, MA.  On or about May 13, 2022, a safe access point was not provided for four employees to access residential house. The break in elevation from ground to first floor deck was greater than twenty four inches in height above ground level.
