29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): 1199 Gilbrook Avenue, Rock Hill, MO: Employer is exposing employees to hazardous conditions where the employees work on roofs higher than 6 feet above lower level without using fall protection systems. On 05/13/2022 employees and their supervisor worked on a residential roof without using any forms of fall protection systems. The employees and their supervisor were exposed to a fall distance of approximately 10 feet 10 inches from eaves of the roof to ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: 1199 Gilbrook Avenue, Rock Hill, MO: Employer is exposing employees to hazardous conditions where employees are permitted to access an upper level without using a ladder extended 3 feet above the landing surface. On 05/13/2022 employees used a Louisville ladder to access a roof approximately 10 feet 10 inches from ground level. The ladder extended 2 feet above the landing surface exposing the employees to fall hazards.
