29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a)Independent Exterior & Repair, Inc. at 1333 North Elk Vale Road, Rapid City, SD 57703: On or about and at times prior to May 11, 2022, the employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment conducted by a competent person. Employees were exposed to fall hazards as a result of conducting roofing activities on a low-slope commercial property without the use of fall protection systems, fall hazards associated with improper use of ladders, and lack of training. This condition exposed employees to a variety of hazards identified on site, including fall hazards in excess of six feet.
