29 CFR 1926.760(c)(6): Safety deck attachments performed in the CDZ from the leading edge back to the control line did not have at least two attachments for each metal decking panel: 201 Federal Street, Portland, Maine - The employer did not ensure that employees placing metal decking used safety attachments to secure at least one panel; over 30 minutes later an employee was injured when the panel shifted and the employee they fell through.
