29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) 15 Lake Rd, Harrison, MT: On or about May 16, 2022, and at times prior, employees were exposed to struck by hazards using the Datsun 6000, Model PGF02, SN PGF02-000677 Forklift to pull bins with a ball hitch attachment throughout the facility. The forks of the forklift had been modified with holes into the forks in order to attach equipment and trailers to them without manufacturers or engineering approval prior to use.
