29 CFR 1910.106(f)(4)(vii): "Hoses and couplings." All pressure hoses and couplings shall be inspected at intervals appropriate to the service. The hose and couplings shall be tested with the hose extended and using the "inservice maximum operating pressures." Any hose showing material deteriorations, signs of leakage, or weakness in its carcass or at the couplings shall be withdrawn from service and repaired or discarded. On or about May 9, 2022, truck loading area; the employer did not ensure that all pressure hoses, couplings, and fittings were inspected at intervals appropriate to the service.

29 CFR 1910.106(f)(6): Sources of ignition. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 �F (37.8 �C), shall not be handled, drawn, or dispensed where flammable vapors may reach a source of ignition. On or about May 9, 2022, truck loading area; the employer did not ensure that adequate precautions were taken to protect employees from the ignition of flammable vapors during the transferring of liquid propane from a storage tank into a tank truck, exposing employees to fire and explosion hazards.
