At the facility - 550 Jessup Road, Paulsboro, NJ 08066: On or about May 3, 2022, the employer did not record a work-related illness of two Certified Nursing Assistants who were COVID-19 positive on the OSHA 300 log or its equivalent. ManorCare Health Services DBA HCR Healthcare, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.4(a), which was contained in OSHA inspection number 1475471, citation number 2, item number 1, and was affirmed as a final order on March 31, 2021, with respect to a workplace located at 550 Jessup Road, West Deptford NJ 08066. Abatement Certification and Documentation is required
