At 109 Mandarin Preserve Road in Jacksonville Florida 32257: On or about 5/9/2022, the employer exposed employees to fall hazards ranging from 6 to 10 feet, in that the extension ladder used to access the roof was not setup at 4:1 ratio while accessing the roof.
