29 CFR 1910.134(k)(5): Respiratory protection retraining was not conducted annually: a. Country Lane Gardens Rehabilitation and Nursing Center located at 7820 Pleasantville Rd, Thornville, Ohio 43076: On or about May 19, 2022, the employer did not provide annual respiratory protection retraining for all employees required to wear respirators including, but not limited to, a Makrite 9500-N95 respirator to protect employees from exposure to SARS-CoV-2 (the virus that causes COVID-19 disease). The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents as well as when in outbreak status at the establishment.

Country Lane Gardens Rehabilitation and Nursing Center located at 7820 Pleasantville Rd, Thornville, Ohio 43076: On or about May 19, 2022, the employer did not provide an annual respirator fit test to all employees who were required to wear a N95 respirator including, but not limited to, a Makrite 9500-N95 respirator to protect employees from exposure to SARS-CoV-2 (the virus that causes COVID-19 disease). Country Lane Gardens Rehabilitation and Nursing Center was previously cited for a violation of this occupational safety and health standard 1910.134(f)(2), which was contained in OSHA inspection number 1479898, citation number 1, item number 1c and was affirmed as a final order on January 5, 2021, with respect to a workplace located at 7820 Pleasantville Rd, Thornville, Ohio 43076.
