On or about May 19, 2022 the employer Elsner Painting & Pressure Washing Inc. at jobsite located at 3885 W. Michigan Ave. Sidney OH 45365, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. One employee using the single aluminum extension ladder to access the roof to perform roofing work was exposed to fall hazards 18 feet or greater to the ground below.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about May 19, 2022 the employer Elsner Painting & Pressure Washing Inc. at jobsite located at 3885 W. Michigan Ave. Sidney OH 45365, did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. One employee installing new roofing sealant material to the structure was exposed to an 18 foot fall to the ground below.
