29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about May 11, 2022, at approximately 1:00pm, an employee reseating the plastic film on a Repack Machine suffered an injury resulting in an amputation. The amputation was not reported within 24 hours to the local Occupational Safety and Health Administration Office. The Austin Area Office was notified of the incident on May 13, 2022 at approximately 11:17am.

On or about May 11, 2022, and at times prior thereto, the employer did not provide clear and precise procedural steps to rectify a misalignment of a newly installed plastic film roll on the Repak machine while in operation or how to control the hazardous energy of the equipment, exposing employees to the hazard of being caught-in the machine.
