29 CFR  1910.106(d)(4)(i): "Construction." Inside storage rooms shall be constructed to meet the required fire-resistive rating for their use. Such construction shall comply with the test specifications set forth in Standard Methods of Fire Tests of Building Construction and Materials, NFPA 251-1969. Where an automatic sprinkler system is provided, the system shall be designed and installed in an acceptable manner. Openings to other rooms or buildings shall be provided with noncombustible liquid-tight raised sills or ramps at least 4 inches in height, or the floor in the storage area shall be at least 4 inches below the surrounding floor. Openings shall be provided with approved self-closing fire doors. The room shall be liquid-tight where the walls join the floor. A permissible alternate to the sill or ramp is an open-grated trench inside of the room which drains to a safe location. Where other portions of the building or other properties are exposed, windows shall be protected as set forth in the Standard for Fire Doors and Windows, NFPA No. 80-1968, which is incorporated by reference as specified in Sec. 1910.6, for Class E or F openings. Wood at least 1 inch nominal thickness may be used for shelving, racks, dunnage, scuff boards, floor overlay, and similar installations.  At this location, flammable liquids were being stored in an area that was not fire resistant.  This exposed employees to fire hazards.

29 CFR  1910.253(b)(2)(ii):Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.  At this location, an oxygen cylinder used for welding purposes, was placed in a common commute area where employees walk through on a daily basis.  The struck by of an employee to the cylinder exposed a caught in hazards.

29 CFR  1910.253(b)(2)(iv):Valve protection caps, where cylinder is designed to accept a cap, shall always be in place, hand-tight, except when cylinders are in use or connected for use. At this location, the acetylene cylinders that are used for welding operations was not secured at the nozzles where the gas is dispensed. This exposed employees to struck by incidents.

29 CFR  1910.253(b)(4)(iii):Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. At this location, oxygen cylinders were stored next to a closet where a mix of other gases and materials were kept.  This exposed employees to fire hazards.

29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. At this location, the junction box had live electrical wiring exposed in an area where employees worked and had daily commute through that area.  This exposed employees to electrical hazards. At this location, there were live electrical wires exposed and an outlet as well.  There were no coverings to protect employees from coming in contact with the live wires.  This exposed employees to electrical hazards.

29 CFR  1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). At this location, employees operated a forklift with no certification or training being provided by the employer.  This exposed employees to struck by hazards.
