29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Worksite:  On or about 5/20/2022, the extension ladder used to access the roof of the building did not have the ladder side rails extended at least 3 feet above the upper landing surface, exposing the employee to a fall hazard of approximately 18 feet.

(a) Worksite:  On or about 5/20/2022, an employee was not protected from falling while working on an approximate 8/12 pitch roof, approximately 18 feet above the lower level.  401 Roofing, LLC was previously cited under the company name Tavares & Sons, Inc., for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1432936, citation number 1, item number 1 and was affirmed as a final order on 11/5/2019, with respect to a workplace located at 2780 Pawtucket Ave., East Providence, RI 02914.
