29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Cayuga Health Project, 260 Tompkins Street, Cortland, NY  13045, on or about 6/3/22:  Employees were installing sheathing to the vertical external surfaces of the building while on a roof section, 12 feet from ground, without fall protection of any kind.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Cayuga Health Project, 260 Tompkins Street, Cortland, NY  13045, on or about 5/11/22:  An employee was installing water and sewer connections along the North-East side of the foundation in an excavation measured at 5 feet 6 inches without a protective system.

29 CFR  1926.1203(a): Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter through consideration and evaluation of the elements of that space, including testing as necessary:   a) Cayuga Health Project, 260 Tompkins Street, Cortland, NY  13045, on or about 5/11/22:  The Superintendent entered a manhole to install a "test plug" for the purpose of checking drain systems in the main structure slab.  The atmosphere was not tested prior to entry, there was no planning for retrieval, and no competent person for confined space was involved in the entry.
