29 CFR  1910.147(e)(1):Before lockout tagout devices were removed, the work area was not inspected to ensure that nonessential items had been removed or ensured that machine or equipment components were operationally intact.  On or about May 20th, 2022 at 3 Lattingtown Road, Glen Cove NY, a hammer was left in the chipper Disc/drum housing  between the feed wheel and chipper flywheel.  Employees were exposed to being struck by a dislodged chipper knife when the equipment was put back into operation.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
