29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures. a) East outer end of the office trailer/container: A 1/2 inch in diameter construction-grade extension cord ran through a 1.5 inch opening at the bottom of a junction box. There was no bushing or other means to prevent the cord's insulation from making contact with the rough/sharp edge of the opening. ABATEMENT CERTIFICATION IS REQUIRED
