29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee(s) used appropriate eye or face protection while exposed to eye or face hazards from flying particles:    On or about 05/19/2022 Garbiel Martinez Roofing, was installing roofing materials on a roof of a residential building located at 14 Swope Park Ct., Wentzeville, MO 63385. Two employees were observed not using eye protection while using pneumatic nail guns.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 05/19/2022, at the residential construction jobsite located at 14 Swope Park Ct., Wentzville, MO 63385 employees were exposed to a fall hazard. Employees were observed actively installing new roofing material, while working on a 8-12 pitch roof approximately 18 feet from the ground.

29 CFR  1926.1053(b)(1):  Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:  On or about 05/19/2022, at the residential construction jobsite located at 14 Swope Park Ct., Wentzville, MO 63385 employees were exposed to a fall hazard.  Employees were exposed to a fall hazard. The exposed employees were observed by the CSHO using the ladder improperly while climbing on the ladder rungs. The crew was using an Aluminum Extension Ladder (no marking) improperly as the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.
