Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards from dump truck tip-overs while operating a tandem axle dump trailer with a full load of wet loam on soft or uneven surfaces and/or inadequately compacted fill: (a) Jobsite at 200 Taft Street, Pawtucket RI: On or about May 19, 2022, the employer exposed employees to struck-by and crushed-by hazards from dump truck tip-overs while operating a tandem axle dump trailer with a full load of wet loam on soft or uneven surfaces and/or inadequately compacted fill.
