29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: St. John's Hospital of the Hospital Sisters of the Third Order of St. Francis located at 800 E Carpenter Street in Springfield, IL. On or about April 20, 2022 and times thereafter, the employer required employees to wear N95 respirators while working near suspected and confirmed positive COVID-19 patients and the employer did not ensure employees were annually fit tested.
