Park Recreational Area, CO-14, Bellvue, CO 80512 - On or about 05/17/2022, the employer exposed employees to a fall from a height of approximately two (2) - three (3) feet, in that; an employee was allowed to use a 10-foot A-frame fiberglass step ladder with visible defects on the steps. The ladder was not tagged with "Dangerous: Do Not Use" or similar language or replaced.  Abatement Note:  Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").
