29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) At 1644 Carpenter Rd, Tifton, GA 31793, on or about April 28, 2022, the employer exposed employees to fall hazards, in that the employer failed to ensure ladders were positioned with the side rails at least 3 feet above the upper landing while employees accessed the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) At 1644 Carpenter Rd, Tifton, GA 31793, on or about April 28, 2022, the employer exposed employees to fall hazards, in that the employer failed to ensure employees were provided with personal fall arrest systems while performing roofing work at heights greater than 9 feet. Anibal Perez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1431484, citation number 1, item number 1 and was affirmed as a final order on November 29, 2019, with respect to a workplace located at Lot 59 Millwood Pass, Marietta, GA, 30064.
