29 CFR  1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  a) In the kitchen area, especially near the dishwashing area and the ice machine, on or about August 11, 2022 and at times prior to, employees were required to walk and work on wet floors, resulting in a slipping hazard.

29 CFR  1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv):  a) Throughout the restaurant, including the kitchen area, on or about May 16, 2022, the employer did not provide training for all employees who used and worked around hazardous chemicals such as cleaning/sanitizing products and compressed carbon dioxide, resulting in a chemical hazard.  Some employees were not trained on i) the methods to detect a release of the compressed carbon dioxide, ii) hazards of the chemical products they used and worked around,  and iii) measures that could be taken to protect themselves from these hazards.
