29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Dairy Farms of America Dairy Brands Ice Cream LLC dba Frostbite Frozen Products on or about April 26, 2022, at the address of 4117 Fitch Rd. Toledo, OH: The employer did not ensure one half inch openings contained on the output conveyor of the Hoyer Machine were guarded in a way that prevented the entry of employees fingers during operation. Employees removing product from the conveyor in this condition were exposed to pinch point hazards where the engaged conveyor meets the fixed transition strip.   b) Dairy Farms of America Dairy Brands Ice Cream LLC dba Frostbite Frozen Products on or about April 28, 2022, at the address of 4117 Fitch Rd. Toledo, OH: The employer did not ensure the eight power actuated cutting blades on the output end of the Hoyer Machine were effectively guarded. Employees operating the machine in proximity of the unguarded blades were exposed to amputation hazards.
