29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.)  On or about May 23, 2022, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a.)  On or about May 23. 2022, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls of approximately ten (10) to twenty (20) feet to ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.)  On or about May 23, 2022, at the above addressed jobsite, employees performing residential roofing operations twenty-one (21) feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.  Manuel Guaman dba MMG Remodeling Inc. was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1452182, citation number 2, item number 1, and was affirmed as a final order on June 15, 2020, with respect to a workplace located at 8515 Fairfield Ln., Tinley Park, IL 60487.  Manuel Guaman dba MMG Remodeling Inc. was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1411874, citation number 1, item number 1, and was affirmed as a final order on November 12, 2019, with respect to a workplace located at 10632 Grey Fox Trail, Naperville, IL 60540.
