29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about May 24, 2022, and at times prior thereto, at the Combi Core area,  the eyewash/shower station was not readily accessible for immediate use when employee(s) had to travel up to 88 feet and the path of travel was obstructed. Employees handled Isocure 706 Catalyst drum containers, exposing them to corrosive material.

29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  On or about May 24, 2022, and at times prior thereto, at the EBBA Iron facility, the employer did not accurately complete the description in column F of the OSHA's Form 300 for year 2022. The information entered did not describe injury or illness, parts of the body affected, and object/substance that directly injured or made person ill.  a. An entry on the 2022 OSHA 300 log form for March 29, 2022, did not describe the specific injury and object that affected an employee's lower back because of a recordable injury incident.  b. An entry on the 2022 OSHA 300 log form for April 4, 2022, did not describe the specific injury and object that affected an employee's right middle finger because of a recordable injury incident.  c. An entry on the 2022 OSHA 300 log form for April 5, 2022, did not describe the specific injury and object that affected an employee's right elbow because of a recordable injury incident.  d. An entry on the 2022 OSHA 300 log form for April 18, 2022, did not describe the specific injury and object that affected an employee's left little finger because of a recordable injury incident.  e. An entry on the 2022 OSHA 300 log form for May 3, 2022, did not describe the specific injury and object that affected an employee's both eyes because of a recordable injury incident.  f. An entry on the 2022 OSHA 300 log form for May 5, 2022, did not describe the specific object that affected an employee's shoulder because of a recordable injury incident.
