29 CFR  1910.178(l): This is applicable to construction employment as referenced by 1926.602(d). The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   The employer failed to protect employees from crushing and struck-by hazards.  This was most recently documented on or about May 23rd, 2022, in Gretna, Nebraska 68028.  The forklift operator had not completed forklift training to include a classroom portion, and written test that meets the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

The employer failed to protect employees from eye and face hazards.  This was most recently documented on or about May 23rd, 2022, in Gretna, Nebraska 68028.  An employee was observed using a pneumatic nail gun without eye protection.

Employees were using damaged extension cords that generated electricity to their power tools that they were actively using on the jobs site.
