29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)	Eric Rew's DBA Rew's Roofing & Siding. On or about May 23, 2022, at the address of 4640 S Detroit Ave. Toledo OH 43614: The employer did not ensure employees were protected from falls while working atop of a 5-12 pitch commercial roof. Employees removing roofing material from atop the structure were exposed to falls of 11ft and greater to the ground below.

29 CFR  1926.1053(b)(1):Portable ladder were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)	Eric Rew's DBA Rew's Roofing & Siding . On or about May 23, 2022, at the address of  4640 S Detroit Ave. Toledo OH 43614: The employer did not ensure employees using aluminum extension ladder to access roof had it side rails extended at least three feet above the landing surface. Five employees were exposed to fall hazards while using ladders to access a single-story commercial structure with a roof eave height of 10ft -11inches from ground below.
