Employees were performing roofing activities on the newly constructed residential home, with pneumatic nail guns without wearing impact resistant eye protection.

An employee was using a 28ft. portable metal extension ladders that did not extend 3 feet above the upper landing while accessing the ladder to get to gain access to the elevated working surface (roof).
