29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:   a) Production Area - On or about June 8, 2022, the employer did not ensure that the energy control procedures that were established, were specific to the equipment such as the Multivac T800, and the ADCO machine, model #15D-10 SS, serial #6226HG, where it clearly outlined the following: a specific statement of the intended use of the procedure (servicing, maintenance, and/or un-jamming), procedural steps for shutting down, isolating and securing the machine to control hazardous energy (electrical, and pneumatic), procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices, tagout devices and other energy control measures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6):  a) Production Area - On or about June 7, 2022, employee was exposed to machine hazards associated with moving parts while  unjamming when the meat is to frozen on the ADCO machine, model #15D-105-SS, serial #6226HG.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work such as unjamming meat from the conveyor line [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.  b) Production Area - On or about June 8, 2022, employee was exposed to machine hazards associated with moving parts while doing a set up for a product line change on the Multi-Vac Machine#3, and the ADCO machine, model #15D-105-SS, serial #6226HG, and to troubleshoot with a problem with the glue.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work such as troubleshooting the glue applicator [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.
