29 CFR 1910.261(k)(13)(i): Broke Holes were not protected by a guardrail that complies with 29 CFR part 1910, subpart D. a. Press Machine 1 - On or about May 24, 2022, employees were exposed to a fall hazard from an unguarded broke hole. The installed single rail did not prevent employees from entering or falling into the broke hole, which would result in a fall to the next lower level. b. Press Machine 2 - On or about May 24, 2022, employees were exposed to a fall hazard from an unguarded broke hole. The installed single rail did not prevent employees from entering or falling into the broke hole, which would result in a fall to the next lower level. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a. The Mill - On or about May 24, 2022, employees assigned to the backtender, 5th hand, and 3rd hand positions were exposed to hazards from cleaning the calendars with kerosene that had not been evaluated as part of the hazard assessment to determine and document the need for PPE such as gloves, FR garments, or face shield. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
