A) All 1 Service, Inc. - 1051 Amboy Ave, Perth Amboy NJ 08861 Power-operated industrial truck (Crown Electric pallet jack) that was not in safe operating condition was not removed from service as required. The control handle did not return to the up position which prevented the safe operation of the brake system. Occurred on or about 4/25/22
