On or about 5/2/2022, each employee operating the Daewoo G25P forklift (Serial #E-120174) to perform preventive maintenance activities such as but not limited to installing of pipes outside a cotton gin wall did not have formal forklift training instruction and evaluation in order to perform the job properly and safely.
